Dear Parents/Guardians,

In the event of a school closure, the Dunellen School District is committed to providing meals to our students.  In order to prepare for such an event, we are asking you to complete the attached questionnaire with details of your child if you would like to accept these meals.  This information will help us place an accurate order from our food service provider. One form per child must be completed (for example, if you have 2 children, 2 forms must be completed).

The meals will be distributed daily in front of Lincoln Middle and John P. Faber Elementary School buildings.  We will provide a school week worth of meals.   Once we have an understanding of the amount of meals that will be provided we will set out a schedule to all parents detailing their pick up time for said meals.

We appreciate your participation in this endeavor.  Be safe and practice good hygiene.
